FAQ

What if my account becomes delinquent?  You will receive three statements regarding your account.  If you have not made a payment or contacted our billing department.  You will be charged a late payment fee of $10 per month. To avoid additional Collection Agency Fees you must pay your statement balance.  If you need to set up payment arrangements please contact our Billing Department.

What if I write a check that is returned to your office unpaid?  Our returned check fee is $20.00 If multiple check are returned on your account, we will require that all future payments be made by cash, cashiers check, money order or credit card.

What if I need a form completed by the office or physician?   You may bring your forms to your doctor visit.  However, the physician may not have adequate time to complete them.  This will be at the discretion of the physician and may require an appointment with one of our Nurse Practitioners.  To avoid delays you can bring them into our Walk-In-Clinic between the hours of 12:30 p.m. - 1:30 p.m. daily, to be completed by our Nurse Practitioners. 

What if I need a copy of my chart?  HIPPA rules allow a patient to review his/her chart.  If a copy of the chart is requested, then a medical release must be signed by the patient.  We do charge a processing fee based on the size of the chart.  You may call our Medical Records department for more information and an estimated cost.

What if I need to cancel an appointment?  We require at least 24 hours notice to cancel an appointment without being charged for the appointment slot. "Cancellations with less than 24 hours notice" are charged a "Cancellation Fee" of $50.00.  This fee is not billable to your insurance company. 

What if I don't show up for an appointment?  All "No Shows" and "Cancellations with less than 24 hours notice" are charged a "Cancellation Fee" of $50.00.  This fee is not billable to your insurance company. 

What Insurances do you accept?  We accept most all insurances.  You can check with your carriers to make sure we are listed as an "In Network" provider.

What if I don't have Insurance?  All self pay patients are charged $50.00 at the time of the office visit (This is like your Co-Pay)  Once you have been seen by the physician and we know what level of service was provided, we will bill you the balance

What if I need a prescription refill?  We encourage patients to get all refills during the office visit.  You may avoid processing fees by having your pharmacy contact us for an electronic refill.  All other requests for refills via mail, fax or phone we do charge a processing fee of $5.00 for each prescription and a maximum fee of $20.00 per call, written request or fax.

Why do you ask to take a photo of me?  For your protection we ask that you allow us to take your photo as part of your chart.  With so many patients having the same or similar names this insures that the chart a physician is viewing matches the patient sitting in front of him/her.
